Care Collaborative

The Care Collaborative is a national quality improvement (QI) initiative to address the critical gaps in HIV care among people with HIV who are not engaged in HIV medical care.
The 18-month collaborative consists of approximately 35 Ryan White HIV/AIDS Program (RWHAP) recipients and subrecipients that have the highest potential for a measurable national impact to improve their organization’s linkage to care, retention in care, and viral suppression rates, while enhancing patient experiences and involving people with lived experience in local QI efforts. This collaborative is managed by the Center for Quality Improvement & Innovation (CQII) and supported by the Health Resources Services Administration (HRSA) HIV/AIDS Bureau (HAB).

Expectations of Participants
Benefits to Participants
  • Participate in monthly QI coaching calls with peers.
  • Present at learning sessions every four months.
  • Submit performance data every other month.
  • Implement QI efforts aimed at improving patient care and experiences.
  • Participate in quarterly reflection sessions to share activities, successes, and challenges.
  • Build capacity for effective QI projects that improve retention and linkage to HIV care.
  • Strengthen local clinical quality management programs that are aligned with Policy Clarification Notice 15-02.
  • Receive support from expert QI coaches, subject matter experts, and people with lived experience.
  • Engage in national networking and peer exchanges.
  • Enhance performance tracking and use national HIV benchmarking data.

Mission of the Care Collaborative
By December 2026, the Care Collaborative intends to meaningfully link and re-engage people with HIV who are out of care in routine HIV medical care to measurably improve their HIV medical care outcomes and experiences with the healthcare system through the application of QI methodologies and tools.
